
Medical Records
The
Health Information Management Department (formerly called Medical
Records) is the keeper of patient health information. Our office
is open Monday through Friday from 8:00 am to 5:00 pm
TO OBTAIN YOUR RECORDS:
To obtain a copy of your records, it is necessary to submit
by mail or in person a signed copy of the Authorization for Release
of Medical Records form. NJ and Federal regulations require a release
to be signed before information can be released. This form can be
viewed then printed from this web site by clicking the following
link.

of Information Authorization Form
If you cannot download this form please call (973) 579-8365 or
e-mail to healthinfo@nmhnj.org
and a copy will be sent to you. Please include your name, date of
birth, and address. By law, no information can be released unless
the form is completely filled out, including specific dates of service,
and properly signed and dated.
NOTE:
If you are requesting material containing sensitive information,
such as mental health, or drug or alcohol abuse records, you will
need to circle or document specifically what kind of records you
are requesting. To obtain a copy of your x-rays films, please contact
the Department of Radiology at

WHAT YOU SHOULD KNOW:
Once the department receives the Authorization Release Form,
it could take up to 30 business days for the record to be photocopied
and mailed. There is no charge for continuum of care, however, for
other purposes there may be a copying fee.
