You can call ahead of time to schedule an appointment, this process will help expedite your service. When calling to schedule an appointment please have the following information available:

· Spelling of first and last name.
· Date of birth
· Social Security Number
· Insurance Information
· Tests that are required
· Name of physician ordering tests
· Diagnosis-reason for test
· Insurance authorizations

Where to go:

The day of your appointment you will arrive at Newton Memorial Hospital main entrance where you will proceed to central registration area.

What to expect:

A Patient Access Representative will register you into our system where you will be asked to verify all information and a copy of your identification and insurance cards will be taken. You will be directed to the appropriate testing area.
